GPt-5.4 requesting permission despite "Run Everything"

When using GPT-5.4 Extra High or High, and only these models, I frequently am asked to allow things to happen, despite settings - Opus 4.6, Sonnet 4.6, Composer 1.5, etc do not do this with no configuration changes . I’m not sure why the model choice would effect this, but it seems too.

A couple quick questions so I can better understand the issue. What exactly is GPT-5.4 asking you to approve? Is it terminal commands, file edits, or MCP tools? A screenshot of the permission prompt would also be helpful.

Let me know based on the details above.

It’s only terminal commands, and seems to be primarily from sub agents. Some of the bigger offenders were go commands for updating packages and such.
