This is something we’ve seen before. After a PR in the same chat is merged, later follow-ups can keep attaching to that merged PR instead of the open one.
Workaround for now: start a new agent for the next change after you merge, or open the open PR directly on GitHub. Same pattern here: Unreliable Github PR linking to change sets.
