Plan mode makes code changes in addition to adjusting the plan

Describe the Bug

Plan mode makes code changes rather than adjusting the plan.

Previously when I would comment during plan mode it would adjust the plan. Now it seems to start making code changes.

Steps to Reproduce

1. Using Gemini Flash, create a “New Agent”, select “Plan”, and add a prompt.

2. View the plan, and then update the plan by adding another prompt with revisions.

3. Observe the plan will be adjusted, but then code will start to be changed despite being in plan mode.

Expected Behavior

Code shouldn’t be changed in plan mode.

I got the same issue today twice, Gemini 3 Pro just straight up implemented the feature first before creating the implementation plan.

The second time Gemini even noticed it’s mistake and reverted the changes:
