Describe the Bug

Since about 3 days ago, I’ve been encountering unexpected behavior where outdated scripts (previously in the allow list) are being triggered automatically during active instructions. The context seems to break mid-process, and while pointing out an error, the system suddenly recalled old file names that are completely unrelated to the current session. It appears there’s a failure in isolating historical context from the current execution. Please review the condition filters and possibly reset/restrict outdated cache or auto-recall mechanisms.

Steps to Reproduce

Just let it do with detailed task yaml with multiple files.
