0.46.3 sonnet-3.7, missing filename references in agent mode

I keep getting this sort of thing, the agent no longer shows me WHAT file that its talking about, so cannot click on almost anything to jump to the code anymore. It’s either “Typescript” or “tsx” or other random generic identifiers.
