The “cancel” button is in the same location as the accept/run button, when reviewing or accepting tool use for multiple successive tool calls it is possible to cancel one by mistake. I suggest that similar to review buttons configuration (Accept / Reject) the cancel button is offset to the left to avoid accidental clicking.
