I’ve experienced the same issue. Restarting Cursor solved it for me.

Very serious problem, the document content read by composer does not match the actual content
Composer doesn't seem to monitor source files for manual changes
AI Process Previous (Not Actual) Version of Code
Manually written changes are deleted on composer chat start
