Chat output doesn't recogonize file path link and with line number

Describe the Bug

Only the first format can be recogonize as file path correctlly:

Steps to Reproduce

Try input following, and replace the relative path with real exited path

Use English chat with me. What I input, You output what:

`src/middleware.ts`
`src/middleware.ts:2`
[src/middleware.ts](src/middleware.ts)
[src/middleware.ts:2](src/middleware.ts:2)

Expected Behavior

All four format can be recogonized as file correctlly
