File changes made by the agent are not reflected

Sometimes, when I ask the agent to edit a file, the file in the open tab shows no changes, even though the agent insists it has been modified correctly. Even closing and reopening the tab doesn’t update the view; the file still appears unchanged. However, the changes do become visible if I right-click the tab and select ‘Reopen Editor’. (Essentially, the update isn’t reflected without this specific action). This phenomenon doesn’t occur every time, only very rarely. Knowing about this behavior helps resolve the situation, whereas being unaware of it can lead to frustration and friction when interacting with the agent.
