GPT-5 High reports edits as completed even when no changes are applied

Describe the Bug

When using GPT-5 High for editing, the model sometimes fails to apply any changes to the file but still reports that the edits were successfully completed.
For example, if the edit request has invalid parameters or other issues, the tool does not modify the code/text, yet the response states that changes were made.

This issue occurs only with GPT-5 High and does not happen with the regular GPT-5.

Steps to Reproduce

Open Cursor and select GPT-5 High .

Request an edit on code/text

In some cases, the model does not perform any actual changes.

Despite that, the output message states that edits were applied successfully.

Operating System

Windows 10/11

Current Cursor Version (Menu → About Cursor → Copy)

Open Cursor and select GPT-5 High for editing.

Request an edit on code/text with parameters that are slightly unclear or complex.

In some cases, the model does not perform any actual changes.

Despite that, the output message states that edits were applied successfully.

Additional Information

I think it wasn’t like this at the beginning — it only happened later, after some change you made.
During the trial weeks, these problems didn’t exist.

Could you post a Request ID with privacy disabled so we can look into the details? Cursor – Getting a Request ID

I need to prepare some examples, because every time this happened, I went back and re-ran the request with a different model.
I’ll look to see if I have something to post here.
