Composer Apply Duplicates Code - Chat Apply Doesn't work properly

I see a trend with this bug. Apply is pretty much the major unique value cursor offers - but its not working. I literally can’t use the editor, its pretty much useless and I’m finding myself having to go back to chatgpt or claude. If any one of the admins can comment on this, whether this is a bug Cursor devs are aware or not, would be great!

Same issue from 2 days. Tried older version also but not resolved. Something changed in the backend.
