FWIW I posted this suggestion for one situation that frequently leads to whole-file duplication. I doubt anyone actually read it, it scrolled off into the noise pretty fast, but eh, here it is again anyway:
Hint: one particular circumstance that can lead to duplicated code from chat
Might help for some of the time.
