No option to "Accept / Reject" applied code, just "Reapply"

Since the latest update, which includes new UI for accept of reject the code from the chat after applied, several times it has happened that I don’t see the “Accept / Reject” options (tick, cross), just “Reapply” (circular arrow), which means that if I want to keep the changes I need to accept one by one in situ.

Hey, is this happening in the Chat, the Composer’s normal mode, or the Composer’s agent mode?

This is happening in chat to me too. I never use composer. It will randomly work too

In the chat, right side… It happens in every session, but not all the time, I have not noticed why sometimes it works and sometimes it doesn’t.
Attached is a screenshot as an example, the code is applied as seen in the left, but I don’t have the option to accept/reject it from the chat (on the right).

It’s like that right after hitting “Apply” (I have not accepted or rejected any individual chunk of code).
