Renaming a file does not update the entry in the context list

Describe the Bug

Renaming a file does not update the entry in the context list.

After deleting the context entry, it still appears in the ‘suggested’ list, and can be toggled to be added and removed until something is typed into the search bar, at which point the cache is cleared and the renamed file can be found at the correct new name.

Steps to Reproduce

  1. Add a file to context (eg blahblahfile.ts)
  2. Rename the file in the file view

Expected Behavior

It should rename the file in the context view.

No - Cursor works, but with this issue
