Review toolbar jumps: Approve / Approve All displaced when reviewing .md files
During agent change review, Approve file and Approve all move depending on file type. For Markdown, preview/view controls appear in the same toolbar area and push the approve buttons, so muscle memory fails and the wrong control can be clicked (especially next to Approve All).
Steps to reproduce
Agent modifies both code (e.g. .php) and Markdown (e.g. .md).
Step through the review queue.
On code files, note approve button positions.
Open a .md diff — markdown view/preview controls replace or shift the toolbar.
Approve buttons move horizontally.
Expected
The Approval buttons should remain fixed when active. Fixed position for primary review actions (Approve file, Approve All, Reject) regardless of file type. Preview controls such as for Markdown should float left, not swapping the approve cluster.
Severity: Sometimes — contributes to mis-clicks on Approve All.
