Updating plans after first plan is written, fails

Describe the Bug

When using Plan mode, after the first plan is written, if I ask for any amendments to be made to the plan before triggering the build, it consistently fails with “Create plan tool errored or timed out”.

After a few times of asking it to update, it sometimes works and updates the plan successfully.

This was on latest version of the IDE, with Sonnet 4.5 and Opus 4.5. Haven’t tried to replicate it with other models.

Steps to Reproduce

  1. Plan mode
  2. Create a plan
  3. Ask the model to make changes to the plan

Hey, thanks for the report. This is a known issue in Plan mode that started in version 2.1.0 and affects plan updates. The team is actively investigating, and it’s being tracked here: Planner broken in 2.1.0

Temporary workarounds:

  • Reload the window and recreate the plan from scratch instead of editing
  • Start a new plan for major changes rather than amending existing ones
  • Manually edit plans in the .cursor/plans/ directory if needed

To help link your case to the investigation, could you please share the Request ID from a failed planning attempt? (Chat context menu at top right → Copy Request ID)

The team has prioritized this and is working on a fix.
