Why does Cursor sometimes ask to confirm "no changes", and how to stop this?

Describe the Bug

Note in the screenshot: it wants me to accept the changes… but there are no changes. All progress is stopped until I confirm. If I don’t notice that it’s stopped in time, the tool times out and I have to start over.

Steps to Reproduce

It happens very commonly when asking to make non-trivial changes with Claude, at the very least.

Expected Behavior

Should just proceed to make the changes. ALSO: when it wants confirmation, it should make a sound to get my attention, but it does not; it only makes sounds when tasks are complete.
