Where does the bug appear (feature/product)?

Failed to Execute Tools / MCP. Only on GPT-5-Codex, using grok fast and grok 4 able to execute tools. not gpt-5-codex

Steps to Reproduce

  1. Prompting
  2. Should Execute tools but its not

Expected Behavior

Able to execute tool/MCP
