Gemini 2.5 Pro Doesn't make any code changes in agent

This has been happening on my end for the last two weeks, mainly with the Gemini model. I’m not sure if it’s a bug or if something is preventing it from auto-applying.

Yes the same was happening to me about 2/3rd of the time and I was not able to isolate why or when this happens. If you found out anything else let me know
