Had a similar issue a few weeks ago with Dev containers and Console Ninja and now this.
Very similar stack trace in output - I was having issues with it and reinstalled it and now lost my theme.

Does this stop you from using Cursor?
The more details you provide, the easier it is for us to reproduce and fix the issue. Thanks!
Hey, I see you’re having trouble installing extensions. Based on the net::ERR_FAILED and OTLPExporterError errors, this might be related to your network or an extension host issue.
A few things to check:
Network Diagnostics
Cursor Settings > Network > Run Diagnostics
Please send the results.
What Cursor version are you on?
Menu > About Cursor > Copy
I want to check if you’re on 2.4.13 to 2.4.14, since those versions had extension host issues.
Workarounds
Let me know your Cursor version and the diagnostics results, and try the workarounds above. If the manual VSIX install also fails, that’ll help confirm if this is a network issue or an extension host issue.
Hi @deanrie
Thanks for the response, it seems to work now.
I have a similar issue with Biome.
The VSIX picker never works - it wont let me pick anything maybe theres a work around on MacOS I’m new to Macs so I don’t know how to force it to be able to be picked
Yes, it looks like when Cursor suggests downloading the extension manually, it downloads it without the .vsix extension. Try renaming the file.
Or try downloading it from the extension’s page: Open VSX Registry
