Plan mode - wrong To-dos progress

Where does the bug appear (feature/product)?

Describe the Bug

After click build, the agent will start runs the plan, while the To-dos shows at the bottom is incorrect

it starts from 0 of 5
when the first is done, it becomes to 1 of 6
then 2 of 7
etc.

Steps to Reproduce

Write a plan, build it, and you will see the To-dos shown at the bottom which will increment in wrong way

Expected Behavior

0 of 5
1 of 5
2 of 5
etc.

The counter bug you’re seeing (both numbers incrementing: 0 of 5 → 1 of 6 → 2 of 7) is related to broader to‑do sync issues in Plan mode that were reported recently.
