Regression: Clicking on a code change doesn't open the code file

Describe the Bug

When an agent makes a code change it shows in the chat. I used to be able to click on the code change and it would take me to the code file where the change happened. This no longer happens anymore resulting in terrible UX.

Steps to Reproduce

Ask an agent to make a code change, then click in the chat window where it changed the code, nothing happens.

Expected Behavior

It should act like it used to and take me to the code change.

No - Cursor works, but with this issue

This significantly slows down my work flow. Being able to check and review code changes made by the agent now takes much longer since i have to manually open the file and find where the change happened.
