Apply_patch failed alot with GPT 5.1

I kept getting apply_patch fail today with GPT 5.1 causing the AI not able to edit the file, but yesterday was fine. Is anyone experiencing it?

With agent mode gpt 5.1

Windows 10/11

Current Cursor Version (Menu → About Cursor → Copy)

Does this stop you from using Cursor

No - Cursor works, but with this issue
