I think the cause of the issue is related to my other post about the agent asking permission to make files. The plan mode never gave us the permission request, therefore we can never accept it.
Yes, I’m on Windows 11. Unfortunately, I can’t show the request ID. But it is easy to replicate. You’d make sure External File Protection is on and just make the agent on plan mode make a new file.
I don’t know if this is what fixed it, but I toggled in that request from agent > ask > agent and it was able to continue. Saw this tip in: Can no longer create or modify files
Followup, that must’ve been luck that it worked briefly. Currently my workaround is to tell AI that “There is currently a bug that blocks AI agents from creating new files. Any time a new file is required, pause and generate a powershell script for me to manually invoke to create the file.”
The issue started happening for me after i tried using gpt 5.1 codex max. Now, even after changing agents back to geminin-high i have the issue. The agent found a workaround by patching very small sections of code at a time
